JAPAN TOURIST /VISIT FRIENDS OR DISTANT RELATIVES

NOTE:  ALL REQUIREMENTS  MUST BE IN A4 SIZE PAPER (ORIGINAL  AND PHOTOCOPY)

  1. Original passport- valid 6 months before departure
  2. Original previous passport if any
  3. Japan visa application form in A4 size paper
  4. 2 pcs. Recent passport size photo with white background
  5. PSA  Birth Certificate – issued with receipt and valid within 1 year   from the time of filing
    If late registration-  need baptismal certificate , school record form 137, school year book.
  6. PSA Marriage Contract – if married  applicant , valid within 1 year from the time of  filing
  7. Daily Schedule in Japan  (TAIZAI YOTEIHYO)
  8. Documents or photos to prove relationship (if visiting friend)
  9. Invitation letter from Guarantor in Japan must have handwritten signature(not applicable to package    tour)
  10. Original Bank Certificate  with Average Daily Balance  and bank statement for the last 3 months
  11. Income Tax Return 2316- with signature of applicant and approving officer
  12. Certificate of Employment – if employed
    If self employed- Business Registration DTI/SEC)/mayors permit/Income tax Return

NOTE: PSA  Birth and Marriage Contract – NO NEED FOR RENEWAL BUT if sponsored NEED to submit to see the  BLOOD RELATIONSHIP

ADDITIONAL REQUIREMENTS :

IF GUARANTOR WHO LIVES IN THE PHILIPPINES WILL SHOULDER PART OF ALL THE APPLICANT’S TRAVEL EXPENSES

Guarantee Letter (must be in Original hand written signature)
Proof of Relationship between applicant and Guarantor

Residence Certificate (JUMINHYO)
Income Certificate from City Hall in Japan (SHOTOKU SHOMEISHO), Bank Certificate (YOKIN ZANDAKA SHOMEISHO) Certificate of Tax Payment (NOZEIHOMEISHO FORM 2), or Tax Return Certificate from Tax Office (KAKUTEINSHINKOKUSHO HIKAE) – with stamped
